Stroke Rehabilitation Survey

 

To: Stroke Clinicians Practicing in Rehabilitation Settings,

We invite you to complete our 2017 update of the Canadian Stroke Rehabilitation Services Resource Inventory (SRSRI).  Kindly help us to identify and better understand stroke rehabilitation services in Canada and see how far we have progressed since the original inventory in 2014!!

Our goal is to capture information about every rehabilitation service in Canada that provides inpatient rehabilitation or outpatient/ambulatory/community-based rehabilitation and stroke recovery services.  Your assistance is greatly needed and appreciated!

The inpatient rehabilitation inventory should be completed online, one per stroke rehabilitation service,  at:

(Note, this is a Canadian-based survey tool which is permissible for use by most provinces)

Please Note:

  • The inventory will take about 15 minutes to complete online once data has been gathered.  The system will not allow you to save and return later so please complete all data at the same time
  • We suggest that you complete with your colleagues, and make this a team exercise where you have the opportunity to review your services and resources.
  • Deadline for completion of the SRSRI is April 3rd, 2017 
  • NOTE:  there is a separate inventory for outpatient/ambulatory/community-based rehabilitation and stroke services which will be available soon.  If you are working in this setting please send your name and contact information to Jessica McQuiggan (Jessica.mcquiggan@heartandstroke.ca) and you will be sent the link as soon as available.

Background:  The Heart and Stroke Foundation stroke quality advisory committee is conducting an environmental scan of stroke rehabilitation services across Canada.  This is being accomplished through a Stroke Rehabilitation Services Resource Inventory.  Our goal is to identify the location, scope and functioning of any type of stroke rehabilitation and community support services being provided and publicly funded or locally supported in Canada – whether:

  • in a designated inpatient stroke rehabilitation program,
  • general stroke rehabilitation services provided in acute care hospitals or stand-alone rehabilitation facilities,
  • rehabilitation or support services offered in an outpatient or ambulatory or community-based setting (including home-based)
  • other setting providing rehabilitation, recovery or support services for stroke survivors and family
  • Note, this inventory does not extend to privately operated and privately funded for-profit services.

If you are involved in providing any type of stroke rehabilitation services or support services during recovery, regardless of location or volumes, kindly complete this inventory. 

The results of this SRSRI will help us better understand the availability and range of stroke rehabilitation services in Canada and in different settings.  Results will be included in our 2017 Stroke Report to be released in June 2017, and for other stroke quality monitoring initiatives.
